From a tragedy, progress - bringing the wisdom gained the hard way in aviation to the medical profession. “This is classic human factors stuff. Fixation error, time perception, hierarchy.” “People have limits, and many of their mistakes are predictable effects of those limits. I’ve seen this too. When I started at my current place, I was told we were dead last in safety. We developed a good independent investigation program, learned a lot, and how to better invest our training and resources. In a year, my plant was number one in safety. Not the first time I’ve seen that happen too. When good people get together, great things happen.

Great result!
